• Review & discuss key lessons from the Geriatric and LTC death review committee at the Office of the Chief Coroner.
• Review & learn relevant clinical practice pearls to assess frailty in the community.
• Review and discuss the medical and social perspective and community resources for elderly patients with dementia.
• Foster interprofessional collaboration in primary care setting to provide high quality care to the elderly
