Dear Sir/Madam,
I am writing to remind you about our newly launched Support Coordination service for older people in the Dublin North City & County area. ALONE, the charity that supports older people to age at home, has partnered with the HSE to expand its support services to all older people living in the community. ALONE has 40 years’ experience in providing this free and confidential service.
Our service aims to respond to and address any issues facing older people, which are impacting on their ability to remain living independently in their own home. ALONE’s Support Coordinators assist older people who live in a range of accommodation types; privately owned, private rented or social housing homes.
We provide support to older people across a range of issues such as:
- Accessing allowances e.g. clothing grant,
- Assisting with applying for entitlements e.g. fuel allowance,
- Addressing cluttered or unsafe living environments,
- Coordinating and managing large grant applications and home adaptions,
- Transitioning between care to home settings
- Linking in with social activities and services e.g. befriending and friendly call services,
- Supporting with miscellaneous financial and other issues e.g. literacy difficulties,
We are already actively working in the area and are looking to enhance our referral pathways. If you are aware of any older people who may benefit from our support, please feel free to pass on our details or contact us about them.
