LEARNING OBJECTIVES
By attending this activity, participants will be able to:
- Identify the impact of transitions in care upon patients’ health outcomes and experiences of receiving care.
- Recall practical knowledge in how to assist older adults experiencing transitions in care.
- Identify the intricacies of various transitions experienced by older adults, including transitions into hospital or long-term care, stopping driving, experiencing losses, and end-of-life.
